Online Booking

Below are steps on how to book an appointment:

1. Click the View Appointment Form button below.
2. Fill-in all required fields in the Appointment Form.
3. Send your appointment details by clicking the Submit button.
4. You will receive an Appointment Number in your email account. Make sure to type your email address correctly.
5. We will email or call you to confirm your online booking/appointment.
6. Present your appointment number to our triage staff on your scheduled visit.
